AWARDS NOMINATION FORM
You must fill out the Awards Nomination Form by downloading the form in MS Word (.doc) or Adobe Acrobat (.pdf) format and sending the completed form via regular mail. This is necessary as the sponsor or person submitting the nomination must provide their signature on the form. in addition, please note that the form has sections which may require the nominee to complete. It is the responsibility of the sponsor to ensure that this form is submitted properly with any supporting documentation. You should make copies of completed materials for your records.
Please note that three letters of recommendation are required for Hall of Fame nominations in addition to the application form. Two of these must be from certified active members of TATS in good standing and one from a physician licensed in the State of Tennessee. See the form for the specific requirements of these letters.
All materials must be submitted by December 1st . Further instructions and where to send materials are indicated on the Awards Nomination Form. |
